Community Living Support focuses on teaching the skills needed for greater independence. This may include cooking, cleaning, managing money, practicing communication, and navigating the community. CLS is tailored to each individual’s abilities and goals, ensuring that your loved one gains the confidence to participate in everyday life more fully.
Alternative Family Living (AFL)
AFL offers individuals the opportunity to live in a family-like home setting when living with their natural family is not possible or ideal. Caring host families provide daily support, supervision, and encouragement, while also creating a nurturing environment where individuals feel accepted and included. AFL combines the warmth of a family atmosphere with the structure needed to promote personal growth and independence.
Respite Care
Every caregiver needs a break, and respite care provides families with that relief. Our respite services ensure your loved one is cared for by a trained professional in a safe and supportive environment. Whether for a few hours or a few days, respite care gives families time to rest, recharge, and take care of other responsibilities while knowing their loved one is secure and supported.
Supported Employment
Supported Employment helps individuals prepare for, find, and maintain meaningful work in the community. We provide job readiness training, interview coaching, on-the-job support, and long-term guidance to help each person succeed. Employment builds not only financial independence but also self-confidence and social connection, allowing your loved one to feel valued and accomplished in their daily life.
Day Support
Day Support programs provide structured, meaningful activities during daytime hours in a group or community setting. These programs encourage socialization, skill-building, recreation, and personal growth. With a focus on inclusion and community involvement, our Day Support services help individuals build friendships, discover interests, and strengthen independence in a safe, welcoming space.
Direct Support Professionals (DSPs)
Our DSPs are the heart of HealthBridge Care Services. They provide personalized, one-on-one support in the home and community, helping with daily routines, personal care, transportation, and social activities. Beyond tasks, DSPs build trusting relationships that give families peace of mind and empower individuals to live with dignity and independence.
